The Department of Justice on Monday unveiled charges against more than 320 individuals in connection with a sweeping, coordinated crackdown on health care fraud schemes that collectively attempted to steal approximately $14.6 billion from federal and private health programs, as reported by The Western Journal.
According to DOJ Criminal Division Head Matthew Galeotti, the scope of the fraud is unprecedented and represents the largest coordinated health care fraud enforcement action in the agency’s history.
The total amount in attempted fraud more than doubles the previous annual record of $6 billion.

“These criminals didn’t just steal someone else’s money. They stole from you,” Galeotti stated.
“Every fraudulent claim, every fake billing, every kickback scheme represents money taken directly from the pockets of American taxpayers who fund these essential programs through their hard work and sacrifice.”
Galeotti also warned that these crimes directly contribute to rising deficits and jeopardize health care access for seniors, disabled Americans, and other vulnerable populations.

The schemes targeted programs, including Medicare and other taxpayer-funded and private insurance systems. Dr. Mehmet Oz, Administrator of the Centers for Medicare and Medicaid Services, emphasized that these crimes were not isolated or amateur efforts.

“These are organized syndicates who are designing to hurt America,” Oz said.
“CMS is probably the largest target of all, responsible for about $1.7 trillion of disbursements. So it’s a big target on our side.”
Oz explained that criminals often obtain ID numbers from seniors and other individuals and then use that information to file false claims. In response, CMS is creating a “fraud war room” to respond more rapidly and aggressively to such threats.

In one of the largest schemes described by the DOJ, at least 20 members of a transnational criminal network allegedly acquired dozens of U.S.-based medical supply companies.
They used those companies to submit approximately $10.6 billion in fake claims to Medicare, primarily for urinary catheters and other durable medical equipment.
These claims were submitted using stolen personal and medical information from over one million Americans in all 50 states.
Another case involved a defendant operating out of Pakistan and the United Arab Emirates.
According to prosecutors, this individual worked with U.S.-based addiction treatment centers to fraudulently bill Arizona’s Medicaid system for around $650 million in substance abuse treatment services.
In yet another fraud ring, five medical professionals in Nevada and Arizona were charged with submitting $1.1 billion in fake claims involving skin grafts to superficial wounds—often targeting elderly patients in hospice care who did not require such procedures.
Attorney General Pam Bondi commended the DOJ’s extensive operation, stating, “This record-setting Health Care Fraud Takedown delivers justice to criminal actors who prey upon our most vulnerable citizens and steal from hardworking American taxpayers.”

She added, “Make no mistake — this administration will not tolerate criminals who line their pockets with taxpayer dollars while endangering the health and safety of our communities.”
The DOJ emphasized that its efforts are ongoing and that more enforcement actions are expected as investigations continue.
